Forms and Fees
Thanks to Theta Chi’s partnership with OmegaFi, all new member and initiation reporting should be done through the OmegaFi Chapter Desktop account. Other forms may be found below:

• Alumnus Initiate Form- to nominate an alumnus initiate
• Transfer of Membership Form- to report an individual member’s transfer of membership from one chapter to another
• Suspension/Expulsion Procedures Form- to report a member who has been suspended and/or recommend a member for expulsion
• Silver Legion, Golden Guard and Diamond Brigade- to honor a member's 25, 50 or 75 years of membership in Theta Chi
