Initiation Request Form (IRF-1)
To be sent to the Headquarters with the initiation fee three (3) weeks prior to the date of initiation. Please submit only one chapter check for all initiation fees - do not send individual personal checks. Checks can be sent to Theta Chi Headquarters, 3330 Founders Road, Indianapolis, Indiana 46268.
Report of Roster Numbers
To be filled out and mailed to the Headquarters within 24 hours after Initiation.

Alumni can use this form to update their information with Theta Chi Headquarters.

Transfer of Membership Form (T-1 Form)
To be used when a member of another chapter requests the transfer of his membership to your chapter. Send the form to his original chapter. They are to fill out the top half of the form and return it to you. Your chapter should then complete the remainder of the form and forward it to the International Headquarters.
Nomination Form for Alumnus Initiate (E-3A)
To be completed and returned along with the proper fees to the Theta Chi Headquarters prior to initiation. The Grand Chapter must give the approval for all alumnus initiates.
Suspension/Expulsion Proceedings Form
Information should be sent to Theta Chi Headquarters. Expulsion information will then be sent to the Grand Chapter of Theta Chi Fraternity.
Petition for Release Form
This form must be completely filled out and submitted to Theta Chi Headquarters if a Brother wishes to be released from membership pursuant of the National Constitution and Bylaws of Theta Chi Fraternity.
