NEW MEMBERS

1. New Member Registration Form (RF-1)
Registration Cards are to be sent with the New Member Fee within ten (10) days after recruiting. (Please submit only one Chapter check for all New Member fees - do not send individual, personal checks).

2. New Member Initiation Request Form (IRF-1)
To be sent to the International Headquarters with the Initiation Fee three (3) weeks prior to the date of initiation. (Please submit only one Chapter check for all Initiation fees - do not send individual, personal checks).

3. Member's Personal Record Form (E-1)
A Member's Personal Record Form is to be filled out within 24 hours after initiation. Remember to include the roster number and the exact date of Initiation.

ALUMNI FORMS

Nomination for Alumnus Initiate (E-3A)
To be completed and returned along with the proper fees to the International Headquarters prior to Initiation. The Grand Chapter must give the approval for all Alumnus Initiates.

Transfer of Membership Form (T-1 Form)
To be used when a member of another chapter requests the transfer of his membership to your chapter. Send the form to his original chapter - they are to fill out the top half of the form and return it to you. Your chapter should then complete the remainder of the form and forward it to the International Headquarters.

ADDITIONAL FORMS
Petition for Release

Suspension - Expulsion Form
To be sent to the International Headquarters for review. If a chapter requests the Expulsion of a member, the form is then sent by our office to the Grand Chapter for approval.
